    I mainly started thinking about this due to settlements and settlers having relatively poor hygiene (usually not out of choice but lack of resources) and how that was a huge vector for the disease in areas like Europe. One example that comes to mind is one town in Fallout 2 that has a quest requiring you to go into the septic hole for the latrines which contains large amounts of feces and rats. Looking on Wikipedia the disease origin was from a specific flee around Central Asia but given the 200+ years and again the poor hygiene of settlements I personally think this would be an interesting scenario to see in a future fallout game.

    So what would have happened if the Nuka World gangs moved in on the Commonwealth before the Sole Survivor was released from cryo-stasis?

    So the year the SS is released into the wastes the year is October, 2287. If I can set a decent date for when the gangs were United I would say that Colter gathered the gangs the date would have probably have been close to 2285-2286; this inference is gathered from in game dialogue where one of the NPC's says that they spent over a year together in Nuka World.

    If I remember Colter's plan, it was to take control the the parks then invade the Commonwealth. Seeing as it took the SS alone (if you side with the gangs) less than a month to clear out every park, Colter could gather a combined force to clear out every park in less than 4 days minimum. This would finally set his plans into motion as he would finally be able to invade the Commonwealth.

    So assuming he did this two months after the gangs assembled so they could get "friendly" enough with each other, he would have most likely invaded the commonwealth around the end of the year, 2285.

    The active factions in the Commonwealth at the time would have been: the Minutemen, the Institute, Super mutants, Diamond City, Goodneighbor, the Railroad, other raider gangs (assuming any of the following gangs were formed at the time, e.g: Forged, Rust devils, Libertalia), Gunners, Brandis' recon team, Vault 81, Atom Cats, Children of Atom and Triggermen

    So how would the commonwealth fare if the Nuka World raiders actually invaded before the SS woke up? And how much more blood would there be?
