What determines if a Nuke is Blinding in the Fallout Universe?

In the Honest Hearts DLC for New Vegas, in Randall Clark's first terminal entry, he states that he killed an older couple out of mercy because they were blinded by looking at a nuclear detonation in Salt Lake City. This terminal implies that they were several hundred miles from SLC, and the effects were that far reaching.

However, in the start of Fallout 4, both the player and their neighbors all look at a nuclear detonation (assumed to be) in Boston, which would be much closer than Randall and the couple were to SLC, however were unaffected by it.

The first scenario is much more true to reality, as the light produced by a nuclear weapon can blind people who look even indirectly at it.

Was this difference caused by the type of bomb, or is there any lore reason explaining this?
