    I've always been fascinated by the pulp side of Fallout lore. As such, I decided to compile a list of every known paranormal and alien event in the games. This list will contain both verifiable events (those witnessed by the player character) and any significant references to otherworldly entities. It will not contain events or individuals whose abilities are explained in-game by the science of Fallout, such as the Master possessing psychic abilities due to experimentation. These events are listed in chronological order.

    I believe I've covered everything, but if I missed something feel free to let me know in the comments so I can update the list. Thank you and enjoy!

    Editing note: After several suggestions, some debate, and serious consideration on my part, I've decided to include in this list every "special" encounter in Fallout and Fallout 2. Despite their questionable canon status, these events may or may not be encountered in the base vanilla game by all player characters (the ability to access them is a dice roll of your Luck stat, meaning even a Luck 1 protagonist has a small chance of encountering them), placing them in the same category as other, more "realistic" random encounters in Fallout 3 and Fallout 4 such as Downed Chinese Pilot (Fallout 3) or Two Faces, One Synth (Fallout 4). Whether you regard them as canon is a purely personal choice until further revelations from dev sources.

    I will also be including the Mysterious Stranger due to his canon case file from Fallout 4 in Nick Valentine's office. However, I will not consider evidence of the player character witnessing him or Miss Fortune as canon evidence of their appearances, as unlike the special encounters above, their presence requires a perk selection and may not be encountered by all player characters.

    I will not be adding any events that occur as a result of taking the Wild Wasteland trait in Fallout: New Vegas for the same reasons as above and because the game clearly warns the player this results in "silly" events and can change aspects of the base vanilla game, which should be taken as canon when possible.

    1500s

    • The Zetan aliens abduct Toshiro Kago, a Japanese samurai of the Oda clan, to Mothership Zeta.

    1697

    • The Zetan aliens abduct Andrew Endicott, a resident of Salem, MA, to Mothership Zeta.

    1800s

    • The Zetan aliens abduct Paulson, an American cowboy, to Mothership Zeta.

    1894

    • The 1894 Empty Quarter expedition, led by Lorenzo Cabot, uncovers archeological evidence of the lost city of Ubar*; a progenitor civilization that predates humanity.
      • Lorenzo's Crown, an ancient artifact whose original name is lost to history, is discovered within a burial crypt. Upon donning it, Lorenzo Cabot gains several supernatural abilities including increased strength, resistance to physical damage, immortality, telekinesis, and telepathy. Reports state Lorzeno's extended use of the artifact induced paranoia and homicidal rage.
      • Other findings included "disturbing geometries, tools not made for human hands, carvings that hint at dimensions beyond our own(...)."

    \It is noted that) Lorzeno's journal states the name of the city is not actually Ubar, but this document will continue to use this name for ease of reference.

    1960s

    • The Clarabella 7, on its return from a low orbit space flight, makes contact with Mothership Zeta. Colonel Hartigan and Holly Barrisford are abducted.

    2041

    • The Zetan aliens abduct Dr. Morrison Rand, the Professor of Anthropological Archaeology at Banfield College, in Humboldt, OR, to Mothership Zeta.

    2064 - 2069

    • Giles Wolstencroft, the Assistant Chief Executive Officer of Vault-Tec, is abducted by Zetan aliens while overseeing the construction of Vault 76.

    2066 - 2077

    • The Zetan aliens abduct several U.S. Army personnel from Achorage, Alaska to Mothership Zeta, including Elliott Tercorien.

    2077

    • The Cult of the Mothman, led by Brother Charles, is successful in its attempts to summon the Mothman.
      • The Mothman, a large, winged beast with a furry texture, is witnessed some years later by multiple Vault Dwellers of Vault 76.
    • The Sheepsqatch, a large, bipedal ram-like creature, murders two teenagers in Toxic Larry's Meat 'N Go.
      • Most theorize the Sheepsqatch to be a farm animal exposed to radiation, though multiple theories state otherwise, hence its inclusion on this list.
    • The Zetan aliens abduct Sally and her unnamed sister, who died sometime later, shortly after the bombs fell to Mothership Zeta.

    2103

    • Vault Dwellers from Vault 76 encounter The Interloper, a massive, multi-armed humanoid figure consisting of roots and vines in Lucky Hole Mine.
      • The Cult of the Mothman is also found worshipping this entity, offering blood and human sacrifices.
      • Known abilities of The Interloper include telepathy (and providing "visions" to its worshippers) and manipulation of nearby terrain.
    • Three alien corpses are found within Enclave Research Facility Site J within Holding Cell Z05 in Appalachia.
    • The Flatwoods Monster, a humanoid creature encased in a purple helmet and protective suit with no legs and large, three-fingered hands, is witnessed by multiple Vault Dwellers of Vault 76 prowling the region of Appalachia.
      • Known abilities include limited hovering via hover-jets attached to its suit, teleportation, and the ability to control other creatures and robots.
      • It is later determined that the Flatwoods Monster is actually a number of Zetan alien agents who were using the area of Flatwoods for experimentation.
    • Wendigos, a gaunt, lanky humanoid with gray skin, are seen throughout Appalachia.
      • Their precise origin remains unknown, though one possible cause is cannibalism (as was the case with Morris Stevens and Earle Williams).
      • When exposed to high levels of radiation, they are known to mutate into a wendigo colossus, a larger version with three heads.
    • Sofia Daguerre, a former U.S.S.A. astronaut, possesses a unique variant of the alien blaster dubbed The V.A.T.S. unknown, which she bequeathed to a Vault 76 dweller.
    • An alien blaster is discovered by Vault 76 dwellers within an unmarked location in the Toxic Valley of Appalachia inside an open safe.
      • A nearby note indicates "these could very well be of alien origin", which could indicate either the alien blaster itself or the nearby skeleton on the hospital gurney.

    • The Vault Dweller encounters an unusual blue call box. When approached, the light on top begins to spin and the box slowly vanishes into thin air, leaving behind only a motion sensor.
    • The Vault Dweller encounters a crashed saucer of extraterrestrial origin, inside of which they discover the body of two aliens, a painting of Elvis Presley on velvet fabric, and an alien blaster.
      • Upon closer inspection the saucer reads "Property of Area 51: Return if found", implying the government's awareness of non-human life.
    • A herd of Brahmin is discovered by the Vault Dweller calling out "Moo!" and "Moo, I say!"

    2241

    • Arroyo's shaman, Hakunin, appears in a series of dreams to the Chosen One to communicate with them.
    • The Chosen One encounters the ghost of Anna Winslow in The Den, whose spirit is put to rest after retrieving her lost locket and burying her bones.
    • Dick Hubbell is contacted by the Zetan aliens. He proceeds to use the earth-shattering revelation of the existence of aliens to form an exploitative cult dubbed the Hubologists.
    • A crashed shuttle named "Adoni II" is found buried in the dirt by the Chosen One, alongside three corpses with strange red uniforms.
      • Upon searching the site, the Chosen One finds advanced hypodermics that do not have the same withdrawal effects of Super Stimpacks.
    • The Chosen One stumbles across a giant, ancient-looking portal. Upon crossing its threshold, they are transported back in time to the year 2161 inside of Vault 13, where they then break the Vault's water chip.
    • While wandering the wasteland, the Chosen One discovers a sentient stone head of the legendary Vault Dweller and proceeds to engage it in a 12-hour long argument about who is actually the Chosen One.
    • The Chosen One discovers the skeletal remains of a large, round-headed alien within the Sierra Army Depot.

    2277

    • The Zetan aliens abduct several new creatures from the American wasteland including ghouls, super mutants, brahmin, and even raiders and slavers.
    • The Lone Wanderer experiences multiple paranormal phenomena within the Dunwich Building, including self-opening doors, flashbacks and visions to pre-War, items being flung across rooms, footsteps heard throughout, and an eerie voice whispering "Alhazred" or "G'yeth" in the underchambers.
      • Within the underchambers of the Dunwich Building is a large obelisk, an alter to Ug-Qualtoth, being worshipped by feral ghouls.
      • The Lone Wanderer may place Krivbehknih, a ritual book, against the obelisk; doing so will ignite it and all the feral ghouls.
      • The Lone Wanderer hears eerie voices similar to those within the Dunwich Building at the ritual site in Point Lookout.
    • Bloomseem Poplar, the Oracle of the Treeminders, successfully reveals events happening across the Capital Wasteland to the Lone Wanderer once every few days.
    • The Lone Wanderer is abducted to Mothership Zeta, where they then rescue many other prisoners and fight their way through the ship, ending with a climactic battle against another unnamed mothership. Once successful, the Lone Wanderer and most of the other crew return to earth. Sally and Elliott Tercorien remain behind.
    • It is possible for the Lone Wanderer to experience visions of Elder Lyons while hallucinating during their trip to the Sacred Bog, even if they have not met him.*
      • This may simply be a bug/oversight on the part of the devs, but as it has not been addressed it was decided it should be included.
    • The Lone Wanderer witnesses an unidentified flying object explode in the sky. The Firelance alien blaster and alien power cells are found in the resulting debris.

    2281

    • The Courier hears faint, ghostly whispers when standing too long near graves.
    • The Forecaster, a young clairvoyant living beneath the overpass of the 188 Trading Post, provides insights, or "forecasts", to wandering travelers, including the Courier.
    • The Courier battles with the Ghost of She in Zion Canyon, a large, flame-covered Yao Guai who allegedly summons multiple doppelgangers of itself.
      • According to Walking Cloud of the Sorrows tribe, the Ghost of She is the result of a Sorrows child entering the Yao Guai's den prior to her naming ritual. Upon being consumed, the child and the Yao Guai became one.
      • It should be noted that the Courier admits to having consumed a datura-derived drug prior to entering the den.
    • Jason Bright, a sentient Glowing One living in Nevada, experiences visions of the Far Beyond, a "place of light and healing" covered in radiation that he says exists at an unspecified destination in space.
      • With the assistance of the Courier, Bright and the Bright Followers board REPCONN A3 rockets and launch to their destination, their fates unknown.
      • Jason Bright is a unique Glowing One, in that his presence does not radiate the Courier and his skin is left relatively intact with only small parts of him appearing bioluminescent.

    • Mama Murphy utilizes Jet and other drugs to induce visions of the future to the Sole Survivor.
    • The Sole Survivor encounters the ghost of Lucy Grandchester, who killed her parents and herself, in the Grandchester Mystery Mansion in Nuka-World.
    • The Sole Survivor witnesses a Zetan UFO crash land in the Commonwealth. Nearby, they discover a hostile Zetan and are forced to kill it.
    • Hugo, a raider, is influenced by an unnamed supernatural force to kill himself.
    • The Sole Survivor experiences visions and flashbacks to pre-War within Dunwich Borers, most notably a flashback of an apparent ritual deep within the site led by an unnamed individual in pastor's vestments and nine, unnamed kneeling individuals.
    • The Sole Survivor experiences visions of shadowy figures and creatures while under the effects of hallucinogens during their attempt to infiltrate the Children of Atom. The foremost vision is that of the Mother of the Fog, describing her as "woman, she was like a living shadow, cloaked in mist."
      • While the Wiki states the Mother of the Fog is simply a reclusive hermit, it is missing multiple citations for this theory and the few notes and pieces of dialogue do not actually cement this belief as the correct or true one.
    • The Sole Survivor finds terminal entries from Sergeant J.P. Renkse in Listening Post Bravo detailing hearing strange phrases over the radio frequencies and banging inside the elevator despite it being locked down; when the holotape recording he sent to Washington, D.C. was analyzed, it was revealed there was nothing on it.
      • Inside Listening Post Bravo is a Yaio Guai surrounded by bones and meat, despite there being no clear entrance into the cave beside the locked elevator.
    • A skeleton inside a crashed ship in Fringe Cove Docks is found by the Sole Survivor to be holding a note with a horrifying implication about its cargo...

    I can't wait to drop these damn mannequins off! The crew is starting to claim that they are hearing weird noises from the cargo, maybe they are just playing pranks on me? Whatever, we are almost there.

    • The synthetic detective Nick Valentine investigates sightings of a man dubbed "The Mysterious Stranger" appearing across the American wasteland.
      • Sightings of the Stranger date back all the way to Shady Sands as "really old rumors" and the New California Republic as "old rumors."
      • Nick Valentine cites confirmed sightings of the Stranger in the Capital Wasteland and the Commonwealth.
      • The Mysterious Stranger is described as a human male wearing an old trenchcoat and fedora who appears and disappears suddenly.
      • Possibilities beyond a supernatural existence are theorized as multiple men donning the same title over decades, a pre-War ghoul with minimal scarring, or even being a synth.
    • The Sole Survivor discovers a sword buried in ashes in the town of Bradberton. Interacting with the sword ignites the ash pile and materializes a number of stimpacks.
    • Dara Hubbell, a direct descendant of Dick Hubbell, rewards the Sole Survivor for their assistance with a Zetan alien blaster she claims was passed onto her by her ancestors.

    Special Mentions

    Cafe of Broken Dreams, Mad Brahmins, King Arthur's Knights, A lone surviving dog, Bridge Keeper, Crashed Whale, and Giant Footprint are all unusual and strange encounters, but ones that can be explained and are not inherently alien or supernatural. For example, Giant Footprint may simply be the resulting footprint of an oversized Deathclaw, and Cafe of Broken Dreams is simply a diner with multiple individuals wearing Vault Suits (as well as NPCs from the original Fallout, who are dated to have survived until after the events of Fallout 2 and as such, could quite possibly have been at that location). Named Feral Ghouls from Fallout 4 was also up for consideration, but can be explained as post-traumatic stress disorder of the Sole Survivor.

    Ghost of She from Fallout: New Vegas did eventually make the cut due to J. Sawyer's comments on the matter being left deliberately vague.

    The Lonesome Drifter, while widely accepted to be the Mysterious Stranger's son by the Fallout community, is not considered a reliable source for the existence of the latter due to his dialogue also being left deliberately vague.

    Edit: I considered the Church of the Children of Atom for some time due to their claims of visions and radiation immunity, but decided against it. The visions are secondhand accounts at best, with details being extremely scarce, and have been known to be faked, and radiation immunity is not unique to the Children of Atom (Marie from The Pitt and Brian Richter prior to his religious conversion), suggesting it is either a natural evolution in humankind or a mutation.

    Was the Synth in the broken mask incident like the Mayor

    When they say he freaked out and shot up a bunch of people, they talk like he was full of wires and plugs and stuff like Valentine and I am curious about that. Was there a period where in the Synths had fake skin and such and just looked like super realistic animatronics or did they find synth components on a body?

    I am rather confused about another detail of Synth bodies. Are the components in the brain? Do we crack open a guys head every time we come across someone in our settlement who is a secret synth?
